Instructions:

1. This form must be completed in full, inserting "Not Applicable" where relevant.
2. This form must be accompanied with the subscription payable and entrance fee (non-refundable) which is equal to the annual subcription. This is determined by the number of employees at the time of application. All cheques should be made payable to "MALAYSIAN INSTITUTE OF MANAGEMENT".
